  • YouTube star Nikocado Avocado tells PEOPLE exclusively that after secretly losing 250 lbs., he got a facelift to repair his "drooped neck"

  • The content creator known for mukbangs — eating large amounts of food while talking to viewers — said he used to hide his neck and chin with camera angles

  • He says he's in "utter amazement" at his new jawline and how well the procedure worked

YouTube star Nikocado Avocado is revealing another amazing transformation.

The content creator, whose real name is Nicholas Perry, stunned viewers on Sept. 6, 2024, with his 250-lb. weight loss, which he kept secret by posting pre-recorded mukbangs (videos in which people eat excessive amounts of food while talking to viewers). Perry's video, titled “Two Steps Ahead" was viewed 50 million times.

Now he's going public about getting a facelift to fix his jawline.

“Towards the end of the weight loss journey, my side profile was just horrible," Perry tells PEOPLE exclusively. "I had no chin at all. It was just this big gobble gobble turkey neck sitting below my face. And that's why I said, ‘You know what? I've got to get it taken care of.’ "

Perry, who has 4.6 million subscribers on YouTube, says he's always had issues with his double chin, even before the weight loss. “I've talked to my followers about how I didn't like my recessed chin. I even have some videos out there called, ‘Say hello to my double chin.’ When I gained the weight and lost the weight, it was excessive.”

That's why he sought the assistance of Dr. Ben Talei, a plastic surgeon at the Beverly Hills Center for Plastic Surgery. Talei tells PEOPLE that Perry was “left with a yard of extra muscle and skin that had been kind of stretched over time and then deflated. The only way to get rid of that becomes surgery,” Talei says.

In the video, Perry shares his pre-op jitters. "I don't want to look like a different person," he says, adding that he knows he's in good hands with Talei.

"I'm doing this for myself, I'm doing this for the violin [he's played for years], I'm doing this for my haters," he says.

On the day of the surgery — as he prepares to get Talei's signature AuraLyft facelift and "weekend neck lift" — Perry is nervous but excited. "I'm very confident that it's going to look good, I'm just worried for the pain," he says. "I'm just nervous. I hope I don't pee myself or poop myself."

Because of Perry's dramatic weight loss, “his laxity was so extensive," Talei recalls. "He has something of a domino effect — meaning if you lift his neck, there's so much laxity that it'll push up into the face and it'll have all this excess face tissue.”

It's why Perry also needed a “brow reset," Talei says. “I had to lift his brow out of the way. And that's what we did to give Niko his jawline back."

Perry recovered well — "my tooth extraction hurt way more," he says with surprise — and was thrilled when the bandages came off. "It's so strong, it's so meaty, it looks like there's an implant," he says after seeing his new jaw. "I've actually cried — twice. Tears of joy."

As for why he decided to get the surgery now, Perry says he was no longer able to conceal the skin on his neck. Before, “I knew how to hide well by positioning the camera in certain angles," he explains. But as he started to plan his Two Steps Ahead tour, he knew “photos would be taken of me from all directions and I'd have no ability to 'hide' my drooped neck.”

"Dr. Ben Talei worked closely with me to schedule my facelift ... before the start of my tour to allow my face to properly heal," said Perry, who is loving the result of his procedures. “It's beautiful,” Perry says. “I’m happy where I’m at.”

As for what’s next for Perry, he will still make videos, just not in the same way. The self-professed "theater kid" tells PEOPLE he originally “decided to make YouTube my stage and I just went ham. I went hard into all of the trolling and the acting, and the drama, the meltdowns.”

He says he will continue to film mukbangs — “I love that type of content, I could talk for hours,” he says — they may look different.

“Instead of having five cheeseburgers, I might do a seafood boil. Instead of having a pile of bacon and cheesy fried chicken. I'll have a nice steak and broccoli.”

And Nikocado's future plans?

"Nikocado could be queen of plastic surgery now," Perry jokes. "What's next? The lips, the nose, hair transplant? I don't know!"

In the meantime, Perry himself will be basking in the "true utter amazement" of his new features.

"Now I actually have a jawline," he says in the video. "I have what everyone else has that I've always wanted."
